An excellent paper will encorperate You have the flexability to describe the case from beginning to end or to focus on making movement in a single session (though you still have to briefly describe your plans for more sessions) Maybe add in a referral to someone else is the appropriate intervention. Write a treatment plan, mock genogram, and an actual dialogue should be included. You should define the goals and changes that need to be made and describe how you meet those in goals.

Demographics: Name, age, sex, race
Presenting problem: Drug history, length of time in treatment/phase of treatment, relapse, sanctions
Treatment plans: Goals of treatment, attendance/participation, progress prognosis (relapse potential) termination plan.

Physical/Medical issues/ taking medication
Psych/Social: Client’s support system
Relationship/Living situation
Family of origin
occupation, economics
What are the transference/countertransferance issues? This is for community college so keep it simple im not very punctual ,First page should explain me daniel the counsoler and my office setting. include a release of information. my clients are jamal he is a 31 year old african american man. his girlfriend is wilma 28 caucasian and pregnant with her first child. Jamal has two other children from a previous relationship who live with them fulltime. the mother of the 2 children is in and out of rehab due to a herion addiction and rarley sees the children. Jamal is in probation and has been sent to see you after getting a dirty drug test. in your first visit jamal indicates that his girlfrind wilma is the problem and is not taking care of the children.
