On the first page write an implementation plan for the health program Healthy Horizons”: A Program to Enhance the Health of Urban Communities,
An action plan is needed for implementation; it needs to include these categories:
Activities and major tasks
Persons responsible for carrying out the activity and tasks, including partners
Location(s) where the activity will take place, including space needs
When activity happens, include a timeline for key steps ( provide a graph or table)
Provide a written 1-2 page implementation plan justification for your program.
Program elements to consider:
Registration
Attendance
Participation
Participant progress
Expenditures and income
Implementation of program activities
Program deviations
A program can be put into action using a pilot test, phasing in, or using total implementation. What implementation phase would work best for your program design?
Planners need to ensure that participants are safe and protected from harm or unethical practices. How will your program make sure that all participants and educators are safe?

Evaluation is an important component of the program planning process. Using the CDC Framework for Program Evaluation in Public Health . design an evaluation plan. Explain why you chose this design and list the strengths and weaknesses of the design. Identify an evaluation of a health promotion program that has been conducted on your topic and identify the evaluation approach that it most closely follows. Evaluation begins with the development of program goals and objectives. Develop an evaluation plan to test your program goals and objectives. For each goal and objective please describe how you will assess and measure. Describe how process, impact, and outcome evaluation could be used in your program.
