The requirement for this Final Assignment is to review all of your Health Dimension Assessments. ( Physical, Emotional/Mental, Social, Environmental/Occupational, Intellectual, Spiritual as presented in the Text and all Modules.)
List each Dimension and all of the attributes that are required for Good Health in each Dimension. (Chapter One).
Make conclusions for each of the Dimensions based on your needs as seen in the assessment facts and findings you submitted in the assigned Modules this semester.
Write detailed plans for improvement and maintenance for now and future years.
Write to show much explanation and evidence of thought. COLLEGE CALIBER PLEASE.
Directions for EACH Dimension. (Use the questions presented here. These questions will be your Guide Questions for this assignment.)
A. Copy and Answer all of the following questions for EACH Dimension. (These may differ some from the Guide Questions presented in Module One, but not by much.)
B. Do not combine Dimensions. (Six Dimensions equals six separate question and answer presentations.)
C. Write a complete presentation using the FOLLOWING Questions and Answers for each Dimension.
(If you answer all of the following questions with much explanation, facts, discussion, assessment reference and evidence of thought, you will have a good grade on this assignment. No generalities, PLEASE!)
ASSIGNMENT DIRECTIONS:
1. Copy and Answer these Final GUIDELINE QUESTIONS FOR EACH DIMENSION OF HEALTH. ANSWER, using facts and much explanation UNDER THE SPECIFIC QUESTION. Number and list reasons separately.
2. Six Dimensions need to be presented separately with the following questions: List/State the Dimension before the Questions.
1. What?
( What behaviors need to be improved or maintained? Refer to assessment scores, outcomes, and information. Be specific.)
State a Goal for these behaviors. Give all specific facts needed. (Example: IF you say “Eat healthy foods”, What will you eat? What will you not eat? Name the foods. If you say “Exercise more” Write the specific exercises, purposes, times, plans ETC.)
2. Why?
(What assessment information from the Modules led you to know this is a health need and concern? Present Behavioral, Cultural and Genetic concerns that may be present. Family Diseases, Family Influences, Family Practices ETC.)
3. How?
(What steps will you follow to make this improvement and to maintain this healthy behavior?
List All of the following:
Costs?
Equipment?
Special Needs?
People?
Places?
4. When?
(When will you begin and what is the timeline for continuance? Write a Daily Plan Diary for at least two weeks that incorporates your change behaviors or maintenance behaviors). Be SPECIFIC.
5. Where?
(Where will this improvement and/or maintenance occur?). Why this place?
6. Who?
(Who will you include in this healthy endeavor or who will you need to help you reach your goals?).
7. Outcomes and goals? Facts and specifics. (State what you anticipate and hope this behavioral approach will do for your Health in the future.)

Answer all of the above questions thoroughly for EACH of the Six Dimensions of Health.

8. How has the information presented in this Health 110/DS Class “opened your eyes” and made you aware of the VALUE of your HEALTH?
a. Which Chapters and Modules and Information made a difference? (Name five or more) Be specific.
b. Explain and present examples and evidence of changes you have already made or are planning to make as a result of the information presented in this Health 110/DS class.
9. If you could change someone else’s life (Increase their Quality of Life and Longevity) with the knowledge you have acquired in this Health 110/DS Class, what are the most important concepts or truths you would convey? (Give five or more).

10. A BillBoard on several Eastern North Carolina Highways reads: “Mental Health Is Health Too!”.
State Five reasons why this statement is true based on the Mental Health Chapter Facts.
11. If you could write a letter to a twelve year old child that would assure that this child would never be a victim of Drug Addiction, Alcohol Addiction, Tobacco/Nicotine Addiction, or Bullying, What would you say in the letter based on the subject facts in the TEXT or experiences of others?
Address each of these four subjects /topics in your letter.
