“Medical Record for Patient with Male Reproductive System Complaint” “Improving Sentence Structure and Clarity in Student Writing”

Create a “Medical Record or Report” for a male patient with a complaint of the Male OR Female Reproductive System. This can be a condition, disease or disorder of the Vagina, Vulva, Testes,                             Penis,  Prostate etc.
Include the patient’s name, DOB and other personal information, date of visits 
Document the complaints, signs and symptoms of the diseases, disorders, treatment,
Include and highlight in bold, at least 5 medical terminology you have learned this week.
List exams or tests done by you the Doctor or Nurse (or whatever profession you call yourself this week) Urologist for example

Be as creative as this discussion board will allow 
Use and highlight the terminology!!!   example : Urologist, rectal, prostatic, seminoma
