“Reflecting on My Error in Prescribing Ciprofloxacin: Considering Patient History and MHRA Guidelines”

Within your presentation you advised the f2 doctor that ciprofloxacin should be given to the patient to treat the acute prostatitis. Considering the potential toxicity of this agent, and the patient’s medical history (having been diagnosed with portal hypertension) and the length of the course of the ciprofloxacin this was considered by the panel to have caused a low level of harm. you will need to complete a reflection relating to the incident, the details of which are provided below.
You are required to write a reflection which addresses the error you made, which could have caused harm to the patient. You will need to reflect on:
what the outcome could be for the patient of providing their ciprofloxacin, – ensure that there is detailed referenced information in this element, and that you pay full attention to how rapidly irreversible side effects can manitest themselves.
how the situation arose such that you did not consider the patient’s past medical history prior to deciding on the treatment approach and consider how the risk to the patient may be altered due to that history.
why you chose to ignore the specific guidance from January 2024 from the MHRA of “Fluoroquinolone antibiotics: must now only be prescribed when other commonly recommended antibiotics are inappropriate”
ensure you read the entire guidance.
why you did not consider the setting and the ability to monitor effectively if giving an alternative medication which would have been effective in the treatment of the patient.

“From Childhood Dream to Future Career: My Journey to Becoming a Pharmacist”

I want to rewrite my personal essay for admission to pharmacy school. I graduated with a bachelor of pharmacy. so I am applying for a doctor of pharmacy now. I already wrote an outline for that. I would like to add more sentences and make it perfect as an admission essay. I am attaching instructions for the essay and the outline I already wrote.
Your personal essay should address why you selected pharmacy as a career and how the Doctor of Pharmacy degree relates to your immediate and long-term professional goals.  Describe how your personal, educational, and professional background will help achieve your goals.  The personal essay is an important part of your application for admission and provides you with an opportunity for you to clearly and effectively express your ideas.
To be a healthcare worker was an intensive dream from
childhood. Caring for others made me satisfied. Since I can understand what’s
happening around me, I have seen my grandfather as an apothecary. Before
learning the word phytotherapy, I witnessed plants healing the diseases of
people. Since that time, I have been watching people leave my home with happy
faces. Then I had a situation to take care of my grandmother after multiple
surgeries. I was the one taking care of her medicine to give it as per the doctor’s
instructions. My grandmother was good at maintaining patient compliance. At
that time, I wondered about how these small pills make magic and how these tiny
pills reduce the pain, cure the condition, and help to heal the wound.
After 12th grade, I decided to get into the world
of medicine. That is why I chose a Bachelor of pharmacy. Studying, analyzing
experiencing pharmaceutical chemistry, analysis, technology, pharmacognosy,
pharmacy practice, and a hospital internship of one month helped to mold me to
yearn for crafting my desires in the world of pharmacy. As a break from the
past, when I had to move to the USA, my first step to move on my desire was to
be a pharmacy technician. Here a drastic change happened. The greatest inspiration
I had from my job was my pharmacy manager. More than being in a first job
learned from every moment that I wanted to become a good pharmacy technician
and pursue my future as a licensed pharmacist from my manager. Pharmacists are
learning new things every day. They are Multitasking, caring for patients,
solving all concerns, giving drug utilization reviews, dealing with insurance
problems, actively listening to patients and their families, and making sure
everyone approaching them is getting proper attention. I admire the enthusiasm,
self-esteem, and confidence that every pharmacist has. Working with a clear
focus helped me to take on new roles as an inventory specialist and as an
immunizer pharmacy technician. The skills and knowledge that I acquired
motivated me and gave me the confidence to apply to pharmacy school. I am
excited about the day that I can be an inspiration for the next generation of
pharmacists as I was inspired by my dear pharmacists all over there.
